Overview

Vascular occlusion is a rare but potentially devastating complication of dermal filler treatments. It occurs when filler material either enters a blood vessel or exerts sufficient external pressure to impair blood flow, leading to tissue ischaemia. If not recognised and managed promptly, this can progress to skin necrosis and, in severe cases, permanent scarring or functional impairment.

Although uncommon, vascular occlusion is one of the most serious risks associated with injectable treatments and requires immediate recognition and decisive management.

How does vascular occlusion happen?

Vascular occlusion may occur via several mechanisms:

The degree of tissue damage depends on the vessel involved, the extent and duration of occlusion, the availability of collateral circulation, and the speed and effectiveness of intervention.

High-risk areas

Certain areas of the face are particularly vulnerable due to their vascular anatomy and limited collateral supply:

  • Glabella — risk of supratrochlear/supraorbital artery compromise
  • Nasal dorsum and tip — limited collateral circulation; high risk of necrosis
  • Nasolabial folds — proximity to facial artery
  • Forehead — connection with ophthalmic circulation
  • Tear trough — relation to infraorbital vessels

Extra caution should be exercised in these areas, particularly when using needles.

Recognising the signs

Prompt recognition is critical. Vascular occlusion can present very differently in its early and late stages.

Early Signs (Minutes to Hours)

  • Immediate blanching or pallor
  • Severe, disproportionate pain
  • Livedo reticularis (mottled skin)
  • Coolness of the skin
  • Delayed capillary refill (>2 seconds)

Late Signs (Hours to Days)

  • Dusky, violaceous discolouration
  • Increasing pain or numbness
  • Blistering or pustule formation
  • Skin breakdown and necrosis

Differential considerations

Bruising or haematoma typically lacks blanching and severe pain. Post-procedural swelling is common and expected. Infection usually has delayed onset with warmth, erythema, and possible systemic features. Accurate differentiation is essential — but when in doubt, vascular occlusion should always be treated as the priority.

Immediate management protocol

Vascular occlusion is a clinical emergency. Treatment should be initiated without delay:

Cease injection immediately and assess and mark the affected area.

Firm massage to disperse product and encourage reperfusion.

Apply warm compresses to promote vasodilation.

Administer high-dose hyaluronidase (for HA fillers) — inject into and around the affected area. Repeat dosing may be required at regular intervals. Early, aggressive use of hyaluronidase is widely regarded as the most effective intervention.

Encourage circulation — gentle tapping or massage; consider low-dose aspirin if not contraindicated.

Consider adjuncts with clinical judgement — topical nitroglycerin paste; oral antibiotics if skin compromise is suspected.

Close monitoring and review within hours.

Visual Disturbance — Ophthalmic Emergency

If a patient reports blurred vision, partial or complete vision loss, eye pain, ptosis, or ophthalmoplegia, this may indicate retinal artery occlusion via embolisation through the ophthalmic circulation. Cease treatment immediately and refer urgently to emergency ophthalmology services. Time is critical — visual outcomes are often poor if not treated immediately.
