In the world of aesthetics, botulinum toxin and dermal fillers are often mentioned in the same breath, as though they are interchangeable. They are not. Both are widely used, both can create beautifully natural rejuvenation, and both are designed to help soften the visible signs of ageing — but they work in entirely different ways.

If botulinum toxin and dermal fillers were people at a dinner party, one would politely calm things down, while the other would quietly restore volume where it has slipped away. Let us untangle the difference.

Botulinum Toxin

A purified protein that temporarily relaxes targeted muscles by interrupting the nerve signals that tell them to contract. When those muscles relax, the skin above them stops folding repeatedly, softening expression lines.

  • Forehead lines
  • Frown lines (the "11s")
  • Crow's feet
  • Bunny lines on the nose
  • Jaw clenching and teeth grinding

Treats wrinkles caused by movement.

Dermal Fillers

Restore lost volume beneath the skin. Most modern fillers are made from hyaluronic acid — naturally found in the body. As we age, we gradually lose collagen, fat, and elasticity; fillers help replace that lost support.

  • Cheek volume restoration
  • Lip enhancement
  • Nasolabial folds
  • Marionette lines
  • Chin and jawline contouring
  • Tear troughs under the eyes

Replaces volume that time has helped itself to.

One softens motion. The other restores structure. And often, the best results come from using both together as part of a thoughtful treatment plan. Because ageing, inconveniently, rarely happens in just one way.

What is Juvederm?

At Radiant, we use Juvederm — a premium hyaluronic acid-based dermal filler range. Juvederm comes in several different formulations, each designed for specific treatment areas. Some are softer and ideal for lips, while others are firmer and better suited for cheeks or jawline contouring. It is widely respected for smooth, natural-looking results, strong clinical research, reliable longevity, and an excellent safety profile when properly administered.

Are they prescribed differently in the UK?

Botulinum Toxin

A prescription-only medicine in the UK. Must be prescribed by a qualified prescriber — doctor, nurse prescriber, dentist, or prescribing pharmacist — following a face-to-face consultation. This legal framework exists to protect patients.

Dermal Fillers

Currently do not require a prescription in the UK. The filler industry remains far less regulated than many people realise — which surprises most patients. There are growing calls for reform and proposals to make fillers prescription-only to improve safety standards.

Why practitioner qualifications matter

In a largely unregulated industry, qualifications are everything. At Radiant, treatments are carried out by a medically qualified prescriber. Karen is an Advanced Nurse Prescriber with a BSc and MSc in Advanced Clinical Practice, and Radiant is registered with both ACE (Aesthetic Complications Expert) and Save Face.

ACE provides evidence-based guidance for managing complications in aesthetics and offers emergency support for practitioners dealing with adverse outcomes. Save Face is a government-approved register of accredited practitioners — an excellent place for patients to verify whether a practitioner meets recognised safety standards.

Which treatment is right for you?

The answer depends entirely on what you are hoping to improve. If your concern is wrinkles caused by facial expressions, botulinum toxin may help. If it is volume loss, hollowness, or contouring, dermal fillers may be better. Often, a combination of both offers the most balanced approach. A good consultation should always guide this decision based on your anatomy, goals, and natural facial movement.

Botulinum toxin and dermal fillers may often be grouped together, but they are very different treatments with very different purposes. Neither is "better" — they simply solve different problems. The real key to beautiful, natural results is not choosing the trendiest treatment. It is choosing the right treatment, in the right hands, for the right reasons. Because in aesthetics, subtle and safe will always age better than rushed and regretted.
